This form is used to request services from the SEASnet Computing Facility. If you are only requesting port activation no charges will be made to the account number you have provided. If you are unsure whether or not a charge will apply please note that you would like an estimate of associated costs in the "request details" field.
If you are requesting work on a computer, it is your responsibility to ensure that you have all your data backed up prior to SEASnet beginning the work. If you cannot backup your data prior to the start of the job, notify SEASnet and we will do our best to backup your data for you before the work begins; however, it is understood that SEASnet will not be responsible for the loss of any data.
DNS Requests
If you are requesting a new
domain name within the ucla.edu domain you also need to complete a
DNS
Request Form. The completed form should be faxed to SEASnet at
x59537. All requests must meet campus domain name requirements
listed in UCLA Policy 111: Registration and Use of UCLA Domain
Names. Failure to read the campus policy (specifically, section
B, part 1C) will result in SEASnet
returning the request to you for additional information. If SEASnet
staff time is required to formulate a request that meets campus
policy you will be billed for our time. SEASnet will process the
request within 1-2 business days after receiving the complete request.
We have no control over how long it takes for campus to respond so please
allow at least two weeks. The more complete the justification for your
request the less time is required for the review process. SEASnet does
not recommend that you advertise your new domain name until receiving
final approval from campus. Be aware that not all requests are granted.
