SEASnet Computer Support Request Form

General Information
If you would like SEASnet to maintain your workstation/PC for monthly fee,
please fill out the following form to authorize the support contract.
The monthly fee is as listed below as of 7/28/2003:
- UNIX workstation: $84
- PC (Windows only): $30
- PC (Linux only): $84
- PC (Windows and Linux): $30+$84 if both OS supported
Policies regarding supported machines:
- By submitting this request for a support contract you are
agreeing to have the machine placed on support for at least
one year.
- Support services will be handled thru staff via
eehelp@seas.ucla.edu.
- If your request for support is for a machine that may not
conform to computer vendor standards we will make every effort
to accommodate the request. However, by completing this request
you are stating that you understand that at any given time if a
security patch or upgrade is required and SEASnet is not
successful in installing the patch or upgrade; the machine will
be removed from support.
- Billing for machines on support for Electrical Engineering are
handled differently from other academic units in HSSEAS. These
additional policies apply to all requests from Electrical
Engineering:
- By completing this request, the faculty advisor is
agreeing to notify the EE department of the correct recharge
ID for the machine.
- By completing this request, the faculty advisor is
agreeing to get prior approval from the department chair
or MSO if the expectation is that the machine on support
will be covered by department funds and not the faculty
member's research funds.
- All negotiations for new requests outside of SEASnet's
current support structure for machines to be supported by
department funds must by done with the knowledge of the
department chair and MSO.
- By submitting this request you are agreeing to all SEASnet
policies regarding supported machines.
